Before Your First Session
There is nothing for you to download here. After we speak, I will send you everything you need for a first appointment. This page is so you know what is coming, and why each piece exists — no surprises, no fine print you meet for the first time in a waiting room.

These describe how I practice, how your privacy is protected, and what you are agreeing to.
- Outpatient Services Agreement
Fees, scheduling and cancellation, confidentiality and its limits, records, and how to reach me. This is the agreement between us.

2What I will ask you to complete
One of these, depending on who the client is. Filling it out in advance means we can spend our first hour on you rather than on paperwork. If a question is unclear, or you would rather talk it through in person, leave it blank — we will go over it together.
- Adult Intake Form
For clients 18 and over, and for individual or couples work. Your history, current concerns, and what you want to be different.

3Only if you want me to coordinate with someone
Needed only if you would like me to speak with a physician, psychiatrist, school, or another provider.
- Authorization to Release or Exchange Information
Lets me share information with, or request it from, someone else involved in your care. One form per person or organization. Entirely optional, and you may revoke it at any time.

Please follow the instructions you receive from me directly. Do not send completed forms by ordinary email. Standard email is not a secure way to send health information, and I would rather you not put anything sensitive at risk on the way to our first meeting.
If you are unsure what to do, bring the printed forms with you and we will handle it at the start of your first session.
You have the right to receive a Good Faith Estimate explaining how much your care will cost. Under the law, health care providers must give clients who do not have insurance, or who are not using insurance, an estimate of the expected charges for services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services. You can ask me for one before you schedule. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

I do not accept private health insurance. Sessions are private-pay, and I provide monthly statements with everything you need to submit for out-of-network reimbursement if your plan offers it. If cost is a concern, say so early — it is a normal thing to discuss and I would rather know at the start.
I do not provide crisis-intervention or after-hours emergency services. If you or someone you care about is in immediate danger, call 911, call or text 988 (Suicide & Crisis Lifeline), or go to your nearest emergency room.
